Before entering the Czech Republic, you are obliged to:
1.
Submit the completed Arrival Form referred to in point III. 3,
2.
Before entering the territory or no later than 5 days after entering the territory of the Czech Republic, undergo a test to determine the presence of SARS-CoV-2 virus referred to in point III.4,
3.
Upon request, at the border or residence check, submit a document on the completion of the Arrival Form (notification) or a written proof that the person is covered by the exemption under this protective measure
